The new responsibilities that come with moving from pediatric to adult arthritis care can be overwhelming for both youth and their families. It’s a time to build new skills, gain confidence, and learn how to take more responsibility for managing health and well-being.

To make the journey easier, world leading paediatric arthritis researchers at McMaster University created three tools – two for youth and one for parents and caregivers. Each tool walks you through what to expect, what’s changing, and how to prepare for a smooth move to adult care.
