REFERENCE GUIDE TO ADULT ARTHRITIS CARE

Supporting a young person as they move from pediatric to adult arthritis care can feel like a big transition for the parent and caregiver, too. Parent and Caregiver Reference Guide: Transition to Adult Care was designed to support youth in their understanding of what to expect and guide them toward greater independence in managing their arthritis care in an adult healthcare setting.

Co-created with parents and caregivers, this Reference Guide covers the most common questions parents and caregivers need answers to for the road to adult arthritis care. Questions range from “what is healthcare transition” and “what is my role in a youth’s transfer of care” to “how can I encourage independence” and “who can I turn to for help.”

In the Guide, you’ll also find checklists, examples, and conversation starters to use at your own pace. It’s not about stepping back all at once – it’s about shifting from doing for a young person to supporting them as they learn to do for themselves.
